Wayne
Wayne, 56, has a vision impairment, ongoing mental health challenges, and a history of substance use. For years, he struggled to survive while sleeping rough on the streets, unable to access stable housing or consistent support. His disability made navigating services even harder, leaving him isolated and without direction. Despite these barriers, Wayne was determined to find stability and regain control over his life.

Susan
Susan is a 66-year-old woman who has always valued her independence and social life. After experiencing a stroke, she faced a difficult recovery, compounded by a history of falls that put her at risk of losing her ability to live alone. While she wanted to stay in her home, her declining mobility and need for daily support made this increasingly challenging.

Laura
Laura, 35, is a funny, bright and bubbly woman living with Cerebral Palsy and an Intellectual Disability. Laura has spent her entire life living in her family home, relying on her mother’s full-time care. As Laura grew older, both her and her mother recognised the importance of fostering her independence while ensuring she could live safely and confidently as an adult.

Kally
Kally is a single mother juggling the needs of four children, aged 6 to 16, each with their own set of challenges. The family’s busy life included coordinating multiple NDIS plans, managing appointments, and dealing with overlapping schedules. With children attending three different schools and each child requiring different levels of support, the mother understandably felt overwhelmed and burnt out, trying to keep up with the complexities of her family’s needs.

Nick
Nick is a 55-year-old man living with Parkinson’s disease, a condition that has significantly impacted his mobility, independence, and daily life. While his NDIS plan provided some funding and support, it fell short of meeting his complex needs, making it difficult for him to receive the care and stability he required.
