


Leah McCann
Leah brings more than 12 years’ experience working across disability, mental health, alcohol and other drug services, youth programs and homelessness supports. She has spent the past five years working as a Specialist Support Coordinator, supporting people with highly complex needs including significant mental health, cognitive and physical disabilities.
Before joining Circle, Leah worked with another Support Coordination company in WA where she supported participants with complex life circumstances including hospital discharges, housing transitions, justice system involvement and end-of-life care. She is known for her warm, person-centred approach and her ability to build strong relationships with participants, families and services to navigate complex systems and achieve meaningful outcomes.
Leah has extensive knowledge of services across Perth and regional Western Australia and works closely with multidisciplinary teams to ensure supports are coordinated, responsive and aligned with each person’s goals, rights and preferences.
