How we can help
Getting help at home can mean dealing with multiple services, appointments and people. Our fully coordinated service gives you the freedom and control of self-management, backed by the support and expertise of a Circle coordinator.
We get to know your routine, what you want help with and what you'd rather keep doing yourself. If something isn't working, you have someone to talk to who knows your situation and can help sort it out.
We explain how Support at Home works and help you understand your care plan and budget, so you can decide what suits you.
We help you choose providers and arrange services around your routine. You're not locked into one company's staff, so you can choose your own workers and services. Depending on your funding package, these might include personal care, nursing, physiotherapy, cleaning, transport or respite.
We follow up with providers, check how your services are going and help you understand what’s been spent and what’s available.
If your health or circumstances change, we help review your arrangements. That might mean organising different services, working with a hospital discharge team or helping you request a reassessment of your funding.
Circle and Trilogy care
Circle works in partnership with Trilogy Care, a registered aged care provider, to offer care coordination through the Australian Government's Support at Home program. You get a local Circle coordinator who knows you, and Trilogy Care looks after the funding and paperwork behind the scenes.
A local person who meets you face to face, gets to know your routine, organises your services and stays in touch. When your health or circumstances change, we help you adjust.
Trilogy Care manages your Support at Home funding, pays your invoices, runs background checks on your providers, and gives you access to experienced nurses for clinical support.
You choose your services and who delivers them. You can see your budget and every invoice online at any time, so there are no surprises.

We’ll explain your options and the costs, with time for questions. If Circle feels like the right fit, we’ll help you get set up with Trilogy Care or work through a change from your current provider.
We’ll get to know your routine and help organise the people and services you’ve chosen. From there, we stay in touch to see how things are going and help make changes when you need them.
